Please print this form and indicate the items to be ordered. If you are sending item for repair or alteration, shipping of these items will be free! Simply include the filled in Order Form along with the Handy Form for your items. When your item arrive we will contact you with the total costs.

If you are ordering supplies only, send the filled in Order Form with your contact information to: Knit Alteration & Design,

230 SE State Route 3; Shelton, WA 98584. We will contact you via email with the total costs including shipping. At that time items may be paid for by check, credit card or PayPal.

If you are unable to print the form, send a note or email indicating the items that you are interested in along with your contact information and we will reply with a total cost.
